If you feel that either yourself, or a family member may be eligible for NHS Continuing Healthcare you can discuss with the health and social care professionals involved in your care. This could be a GP, Social Worker or a District Nurse. If they feel an application would be appropriate they could then complete the Checklist Screening tool, if they have had the appropriate training, and send to the NHS Continuing Healthcare team.
Alternatively, it is also really simple to request for a NHS Continuing Healthcare assessment to be completed by the team at WHCCG.
If you would like to request for a Checklist screening tool to be completed by the WHCCG team a request will need to be made in writing as below:

Within this written request please provide the following details for the individual being referred:
- Full Name
- Date of Birth
- NHS Number (if available)
- Current full address and postcode
- GP Surgery
- Name, full address and contact number of the individual’s next of kin
Having these details will allow the team to process the application as quickly as possible.
Once the request has been received by the WHCCG team one of our administrators will be in contact as soon as possible to arrange for the Consent and NHS Continuing Healthcare Checklist screening tool to be completed.
